Passport Checklist

If you are planning a vacation outside of the US, you will need a passport to leave and reenter US borders. You will need the following items in order to obtain a valid US Passport:

Passport application: Form DS-11 (required for first time applicants)

Proof of citizenship

  • US Birth Certificate (certified)
  • Certificate of Citizenship
  • Consular Report of Birth Abroad
  • Naturalization Certificate
  • Certificate of Citizenship

Passport Photo

  • 2" x 2"
  • White Background
  • Full Color
  • 25-35 millimeters from bottom of chin to top of head
  • Directly from front
  • Photo must be less than 6 months old

Identification (plus photocopy of front and back of ID at the time you apply for your Passport)

  • Valid Driver's License
  • Military ID Card
  • Valid US Government ID Card
  • Previous passport
